Hours
Office hours are Monday through Thursday, 8 a.m. to 3 p.m. We are closed on Fridays. Please call 410.580.2880 to schedule an appointment.
Office and Financial Policies
Our office accepts a variety of insurance plans in an effort to accommodate our patients. We ask that you provide us with a copy of your current insurance card at the time of your visit or with your pre-registration packet. We also request that if your insurance requires a referral from your primary care physician, that you arrange for it to be sent to our office prior to your visit. It is our goal to assist you in any way we can. However, ultimately the patient will be responsible for the payment of any services not covered by your insurance carrier if a referral is not obtained or if your insurance carrier considers the visit or procedure a non-covered service. We also request that all deductible or co-pay/co-insurance which YOU are responsible for be paid at the time of your visit. Please consult our billing staff before treatment is rendered.
If a referral is necessary and you come to the office without a referral, or we are waiting to receive it, we may ask you to sign a waiver before seeing the physician.
Fees
The office visit fee covers the examination, consultation, and writing of prescriptions for appropriate medications for your skin condition. Some skin diseases or conditions may require more extensive evaluations or consultations. Surgical fees cover the surgical procedure. With certain procedures however, there may be an additional charge for surgical supplies. Additional fees may be assessed for diagnostic tests, such as tissue examination for diagnosis of bacterial and fungal infections, pathological diagnosis of specimens, blood drawing, and photography. Please check with our staff if you have any special concerns regarding any of these additional costs.
Payment
- Our office participates with many health insurance plans.
- If we DO participate with your insurance plan, you will still be responsible for any deductibles, co-pay/co-insurance, or charges for non-covered medical or cosmetic examinations and/or procedures. Payment is requested at the time of service.
- If we DO NOT participate with your insurance plan, we request that all fees be paid at the time of service. A paid in full receipt will be given so you can file with your insurance company.
- For your convenience, we accept VISA, MasterCard, Discover, personal checks, money orders and cash for all payments.
Minor Patients
Patients under the age of 18 must be accompanied by a parent or guardian at the time of service. All fees, co-pays/co-insurance are due at the time of service in accordance with our office policy.
Cancellation and Missed Appointment Policy
If, for any reason, you cannot keep your scheduled appointment, please call 410-580-2880 with at least 24 hours' notice. We reserve the right to charge for missed appointments or appointments cancelled with less than 24 hours notice. Because of the high demand for dermatology services in this area, if a patient fails to give proper notification or misses 3 appointments, the patient may be unable to make future appointments with the physician in the office.
Past Due Accounts
Accounts are considered past due after 30 days. Any delinquent accounts referred to our collection agency will be subject to interest at the rate of 1 ½ percent per month. If your check is returned from the bank, a service charge of $35.00 will be charged to your account.
Medical Record Copying Policy
If medical records are requested by you, any of your other physicians, or an insurance company, you will be billed at the following rate in accordance with the Maryland State Law: A health care provider may require an authorizing person, who requests a copy of a medical record, to pay the cost of preparation, copying, as well as cost of postage. A preparation fee and a per page charge as well as postage will be required payment for medical record retrieval and preparation/delivery.
There may be occasional schedule changes due to holidays or inclement weather. When in doubt, please call the office prior to coming for your visit.
For all prescription refills, routine appointments, and non-urgent questions, we request that you call during normal office hours.
Please arrive on time for all your appointments, and 15-20 minutes early for a procedure.
If you have an urgent problem during non-office hours, please call the office and follow the instructions on the recorded message.
